DNA Collection Kit Instruction

Important Notes:

• Please ensure do not expose the FTA card to any contact including saliva.
• Always wear gloves when handling FTA to avoid contamination of the Card.
• Fill up the request form according to name’s given in the identity document.

1. Arrange lancet and FTA card.
2. Write the name of the person being tested on the FTA card in the area indicated.
3. Carefully remove the round safety cap from one of the blood lancet.
4. Begin collection process by applying the alcohol swab on the index finger area.
5. Gently pressing the lancet into the index finger.
6. Gently squeeze the finger until a small blood droplet forms.
7. Apply the blood spot firmly to the FTA card within the circle area.
8. Repeat until three separate blood spots have been added to the FTA card.
9. Allow FTA card to dry for 10 minutes and insert in the envelope. Ensure the envelope is sealed.
10. Complete the Blood Collection Form.
11. Package the samples with the Blood Collection Form and Request Form in the return envelope and ship immediately through registered courier.
