Required Documents

• Persons requesting the test are required to complete the Request Form and attached a copy of their identification documents (identity card/passport/birth certificate).
• For persons below 18 years old, consent for the purpose of this testing must be given by the legal parent (as stated on the original birth certificate or statutory declaration), or by the legal guardian. A copy of original documents to verify the legal parent/guardian must be attached.
• Photographs of the persons undergoing this test must be attached.

Payment

• RM1,500/ test; consists of 2 samples (father/child or mother/child)
• RM200/additional sample
• Full payment must be made to USAINS Tech Services Sdn. Bhd. prior to commencement of the test
• Payment can be made by cash or Credit/Debit Card.

Test Report

• The Test Report will be produced within 14 working days from the date the samples are received by the laboratory.

Terms & Conditions

• Person undergoing this test must NOT have received any blood transfusions in the last 3 months.
• The samples will be disposed within 3 years upon submission of the Test Report.
• HID/DNA unit reserves the right to select the appropriate method(s) for the test(s) requested.
